1. Dependent Visa (Dependent of spouse or parent working in Thailand)

1. Visa Application form duly filled out.

2. Applicant passport valid not less than 6 months. The validity of 18 months is

    required for one-year visa application.

3. Copy of passport.

4. (4x6cm.) Photograph(s) of the applicant, taken within the past 6 months.

5. Original and copy of official marriage certificate (For spouse).

6. Original and copy of official birth certificate (For children).

7. Documents required from company in Thailand:

v   7.1 Letter indicating applicant’s name, passport number and the relationship with company’s employee, requesting for   Non-immigration Type ‘O’ visa.

v  7.2 Copy of the company’s registration.

v  7.3 Copy of the company’s list shareholders.

v  7.4 Copy of the company’s latest balance sheet.

v  7.5 Copy of the company’s registration of value-added tax (VAT).

        8. Copy of the work permit (if any).

2. Dependent Visa (Dependent of spouse or parent possessing Thai nationality)

1. Visa application form duly filled out.

2. Applicant passport valid not less than 6 months. The validity of 18 months is

    required for one-year visa application.

3. Copy of passport.

4. (4x6cm.) Photograph(s) of the applicant, taken within the past 6 months.

5. Original and copy of official marriage certificate (For spouse).

6. Original and copy of official birth certificate (For children).

7. Copy of Thai identity card (of spouse or parent).

8. Copy of Thai House registration (of spouse or parent).

9. Bank Statement with a minimum deposit of 100,000 Bath

NOTE

**Company’s document shall be certified by authorized director with seal of the company**

**Documents in foreign languages must be translated into Thai or English and should be notarized by notary organs or applicant’s diplomatic/consular mission.**

**Upon application time make sure you do not have any valid Thai visa on your passport**

Single Entry:           RM220 (Spouse without work permit)

Multiple Entries:      RM550 (Spouse with valid work permit more than 7 months or Thai Spouse)

Gratis:                     Malaysian (fee exempted).

Visa Collection:       14.30- 16.00pm (on the next working day)
